In this study the total of ٤١٥ cases of cutaneous leishmaniasis was evaluated from Dermatology hospital and General clinic in Damascus from February ٢٠٠١ to February ٢٠٠٢. Most of these cases was reported in Damascus and its suburb, and when we ma de the direct microscopic test, amastigotes were detectable in ٢٧١ cases, and the positive cases was increased to ٣٦٤ cases by realizing the parasite culture into diphasic N. N. N. media, which confirm the diagnosis.
Through this research, case studies were reviewed which reviewed the dermatology department at Tishreen University Hospital in Lattakia and specialized centers for leishmaniasis and parasitic diseases in Lattakia and Tartous governorates from the beg inning of April 2018 until the end of December 2019. Where Syria is one of the areas endemic with cutaneous leishmaniasis, as we have noticed in recent years the number of skin infections has increased dramatically in all Syrian governorates, which has made leishmaniasis an important health problem that warrants study. In order to isolate and determine the type and strains of the parasite responsible for skin injuries in Syria, in order to develop a plan to combat this disease and limit its spread. Microscopy and transplantation methods are the gold standard for diagnosing cutaneous leishmaniasis, especially in poor areas where this disease is common. Through this study it was found that the age group most exposed to infection is between 16 - 40 years and that the highest incidence was among males. As for the distribution of skin lesions, the highest percentage was recorded on the face, upper and lower extremities.
In the present study, we tried to compare the sensitivity and the specificity of the rK39 strips and DAT, to serodiagnose the visceral leishmaniasis disease in some endemic villages in south of Syria, in order to apply the best and the easy test i n the epidemiological studies, for serodiagnosis this disease not only in symptomatic patients but also in asymptomatic and suspected cases in order to treat them early and rapidly.
The diagnosis of visceral leishmaniasis (VL) remains problematic for physicians in Syria, because the appearance of this disease is new and its clinical criteria are similar to Malaria, Tuberculosis, and Toxoplasmosis symptoms. For this reason, we used the specific antigen (rK39) of Leishmania donovani complex, in order to certify the appearance of visceral leishmaniasis in some patients. Then we followed the infected cases with VL during treatment (15 days) and after treatment (10 days) with Glucantime®. We noted an improvement in clinical criteria and a significant decrease in the parasite numbers as well as in the specific L. donovani antibodies.
Patients infected with different species of Leishmania in Syria develop specific antibodies. For this reason, we have developed an immunodot assay for the serodiagnosis of cutaneous and visceral leishmaniasis, which utilizes antihuman IgG conjugat ed with peroxidase (HRP) as the visualizing agent.
