the leg ulcer is the common disease, it distributes widely in the world. it affect both genders, and any age. it strongly related to the vascular diseases, its most complications are infection and contact dermatitis. the management of the ulcer is depended to the etiology, and regeneration ulcer using different drugs, the surgical procedure is rarely taking place in managements ulcers. the duration of the study was february 2010 and june 2011. the percentage of leg ulcer among patients of dermatology clinic was 0.32%, and 5% of vascular surgery patients. the main age was 48 years. the most common causes of leg ulcer was chronic venous insufficiency (34%), and arterial (22%). the majority of the complications are contact dermatitis and secondary infection. therapeutic study had included drugs helping regeneration (32 ulcer), the best result was using Mebo and Silver Leaf, the percentage of healing was approximately 99% along 8 weeks.

Background: The defects in the leg is one of the most important challenges facing reconstructive surgeons because of the lack of soft tissue covering the front surface of the leg and the lack of blood perfusion in the lower third of the leg. The mu scle and fasciocutaneous flaps are one of the most important reconstruction options as there have been no local studies about this method so far. Objective: Evaluation of methods of reconstruction the defects of the soft tissues of the leg using the muscle flaps and fasciocutaneous flaps and comparing them with the international studies. Methods: The study included 25 patients undergoing transposition of the gastrocnemius and / or soleus flaps, sural flaps for treating soft tissues in the leg, from 2015 to 2017 in both Al-Asaad and Tishreen University hospitals in Lattakia , gathering information on the etiology the lesion, the time of reconstruction, the flap used to cover the lesion, and the function of the gastrocnemius-soleus unit, and the following Sensory disorders.
Pancytopenia is defined by reduction of all three formed elements of blood below the normal reference . It may be manifestation of a wide variety of disorders , yet there exist few published assessments of the frequencies of various etiologies , an d these frequencies exhibit substantial geographic variation . This study was carried out to investigate for and to identify the causes of pancytopenia , to find out the frequency of different causes , to determine the incidence of pancytopenia in relation to sex and age and to compare our findings with those of other similar studies in different countries .This was a prospective study conducted in Al-Asad and Tishreen academic hospitals in Lattakia city , Syria , ovar a period of one year . A total of 113 patients with the diagnosis of pancytopenia were enrolled in the study All patients underwent a detailed medical history and full physical examination followed by blood sampling for the investigations i.e. complete blood count with peripheral film , erythrocyte sedimentation rate ( ESR ) , liver function test , PT , HBsAg and Anti- HCV , ultrasonography of abdomen . All patients underwent bone marrow aspiration and trephine biopsy for some patients. A definite female preponderance was observed , 59 were female and 54 were male. The majority of cases were encountered in the age group of more than sixty years old .Infiltration disorders were the commonest cause that was observed in 38.1% followed by infections in 22.1% , megaloblastic anemia 12.4% , myelodysplasia 11.5%, aplastic anemia 7.1% , myelofibrosis 3.5% , hypersplenism 3.5% and systemic disorders 1.8% Detailed clinical history and meticulous physical examination along with baseline hematological investigations provide good information and help in systematic planning of further investigations to diagnose pancytopenia's cause.
In this study the total of ٤١٥ cases of cutaneous leishmaniasis was evaluated from Dermatology hospital and General clinic in Damascus from February ٢٠٠١ to February ٢٠٠٢. Most of these cases was reported in Damascus and its suburb, and when we ma de the direct microscopic test, amastigotes were detectable in ٢٧١ cases, and the positive cases was increased to ٣٦٤ cases by realizing the parasite culture into diphasic N. N. N. media, which confirm the diagnosis.

The initial disease is characterized by increased vascular permeability due to a breakdown in the blood-retinal barrier BRB, causing macular edema DME, with a progressive vascular occlusion and retinal neovascularization which are secondary to ischemia and oxidative stress. Laser photocoagulation and vitrectomy only target advanced stages of disease. However, despite laser treatment, patients with DME experienced gradual loss of vision. Intravitreal triamcinolone IVTA reduces the breakdown of BRB and down-regulates the production of vascular endothelial growth factor VEGF. IVTA may moderately but temporarily improves visual acuity in cases of DME. Agents that attenuate VEGF action such as bevacizumab are expected to reduce permeability and neovascularization. Intravitrealbevacizumab IVB reduces macular edema secondary to central retinal vein occlusion, vascular permeability and fibrovascular proliferationin. Calcium dobesilate CD is a potent antioxidant, slows vascular proliferation.
