A-60 years old woman, did cardiac consulting for hypertension Interrogation: The patient is asymptomatic she has the following cardiac risk factors Smoking (1) Packet / day for 40 years and arterial hypertension for 20 years ago , uncontrolled she has not surgical or medicals antecedents Cardiovascular examination: BP: 14,5\90, pulse : 80\ M, No murmurs no additional voices ,peripheral pulse are palpable, no edema , not congestion jugular . Twelve-lead electrocardiography showed sinus rhythm,there are not troubles of repolarization, no of conduction Chest radiography showed normal cardiac silhouette, blood tests are normals Echocardiography throw chest : showed the presence a mass in the left atrium, measuring 4 x 3.5 cm lobed ,smooth heterogeneous, attached to the atrial septum , it has broad base extends from the fossa ovalis to the roof of the left atrium beside superior vena cava to the right side Echodppler throw esophagi does not appear more data, but it showed absence of anthor tumors in other cardiac cavities Cardiac catheterization: showed the presence of multiple aneurysms on the coronary arteries with stenosis of 50% on the first segment of the anterior descending coronary artery(LAD) The patient has been transferred to the Department of Cardiac Surgery, where the tumor was removed successfully and Histopathology showed that it is an aterial myxoma ( benign tumor)

Myxoma comprises 30 -40 % of all benign cardiac tumors in adults. And it occur about 0.5 – 1 for each million in a year . Myxoma occur in any chamber of the heart . but have a special predilection for the left atrium. Surgical resection is the on ly effective therapeutic option for patients with cardiac myxoma . It may be done with any other cardiac surgery ( valve replacement or CABG ) . surgery should not be delayed because death from obstruction to flow within the heart orembolization may occur in as many as 8% of patients awaiting operation . New studies show a good result for the patients who undergo the surgical resection for the myxoma . and low percentage of mortality .
A study was conducted in Al-Assad University Hospital in Lattakia during the period February 2012 and February 2013 the study included a random sample of 97 patients (males and females) divided into three groups: the first group consisted of 16 patie nts with left atrial enlargement with no atrial fibrillation, the second group included 70 patients who had left atrial enlargement with atrial fibrillation, and the third group of 11 patients had atrial fibrillation with no left atrial enlargement. In the study the ECHO Doppler was used for measuring left atrial volume index (LAVI) through the Simpson method, this is in order to assess the role and relationship with the occurrence of atrial fibrillation. It was found out that three patients out of (16) from the first group about 18.75% developed an atrial fibrillation after one year of follow-up, which means that the left atrial enlargement gave a predication of the occurrence of atrial fibrillation. The continuation of atrial fibrillation was also noticed together with its lifetime with an increase in the size of left atrial after one year of following-up the patients of the second group. 3 out of (11) patients from the third group had an increase in the left atrial volume index at the end of the study 45.45% (P<0.001), Accordingly the atrial fibrillation could cause left atrial enlargement.Even though the left atrial enlargement may cause atrial fibrillation and vice versa according to thisstudy, the study shows the possibilities of existence of other variable factors related to atrial fibrillation whichplay a role in the occurrence.
Surgical ablation of atrial fibrillation (AF) has been gaining ground as the preferred method of treatment of this potentially lethal arrhythmia affecting 1/3-1/2 of patients with mitral valve diseases. It is specially suited for patients undergoi ng an operative procedure. Between Dec 12,2002 and Dec 29,2003 ,fifty five operative patients with chronic AF were subjected to AF ablation with microwave energy at Damascus University Cardiovascular Surgical Center . Thirty five patients were female; age ranged between 20 and 72 years. All of them had chronic AF.
