Extracranial carotid artery aneurysm (ECAA) is extremely rare, accounting for only 0.4-4% of all peripheral artery aneurysms. Sir Astley Cooper is credited with the first successful operation for ECAA in 1808. These aneurysms are of interest because they have diverse etiologies and present diagnostic and therapeutic challenges. The symptoms of ECAAs vary according to their location, size, and etiology. The presentation of such aneurysms may vary from an asymptomatic mass in the neck to the development of a permanent neurological deficit as a consequence of micro emboli arising from within the aneurysmal sac. Although most carotid artery aneurysms are caused by atherosclerosis, other common causes include trauma and infection. Carotid artery aneurysms are usually diagnosed by means of ultrasonographic scanning. Additional diagnostic testing -by computed tomographic angiography, or magnetic resonance imaging- can lead to more accurate information on the aneurysm’s size and its relationship to surrounding structures. We herein report an extra cranial carotid artery aneurysm detected in a 46 years old patient with complaint of neurologic Symptoms. Because aneurysm located in the distal ICA, the treatment involved stent graft deployment. There has been no complications noted during the 10 months follow up after treatment. In conclusion, Extra Cranial Carotid Artery Aneurysm is a rare condition and has high rate of complication with conservative approach.

Anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA) is a rare congenital anomaly. It presents predominantly in infancy with features of myocardial ischemia or cardiac failure and may be mistaken for common pediatric cond itions such as colic, reflux or bronchiolitis. With early surgical correction the prognosis is good. In this report we present unusual case of a 5-year-old female with an anomalous left coronary artery arising from the pulmonary artery (ALCAPA) presenting with chest pain,dyspnea and palpitation during physical exertion.Trans-thoracic echocardiography and coronary angiography revealed an anomalous origin of the left coronary artery with mitral regurgitation . She subsequently underwent successful surgical technique where the left internal thoracic(mammary) artery was used to revascularise the left main coronary artery which is considered as the first unique procedure in children. Excellent surgical results were obtained and the patient was discharged from the hospital without complaints.

Background& Objective:Vascular rings are unusual rare congenital anomalies .Its incidence is less than 0.2%.Most of these defects are discovered early in infancy or childhood .They result in complete or partial encirclement of the trachea and/or esop hagus .Some vascular rings are associated with other congenital heart defects, others may be isolated deficits. We present here eleven cases which we have encountered since the mid of 2009.These cases were managed surgically with good results .Through this study, we aim at demonstrating how to treat these cases surgically and to clarify the best approach for each case. Materials & Methods: A retrospective study at Damascus University Cardiac Surgery Hospital in the period between 2009 to 2010 was conducted.11 patients with diagnosed vascular rings underwent surgical repair. Results: Ages ranged between 1.5 months to 5 years with almost equal sex distribution. Most complaints were respiratory. Vascular rings include double aortic arche, aberrant left Subclavian artery and aberrant right Subclavian artery .Associated defects were in54% of cases. No complications were noticed postoperatively or in the follow up period which extended between 7 and10 months' .Remarkable relief and symptoms release were achieved postoperatively. Conclusion: Surgery affords excellent long-term resolution of symptoms. Early diagnosis and surgery are imperative to reduce the long-termsequels.
