Quantifying Graft Detachment after Descemets Membrane Endothelial Keratoplasty with Deep Convolutional Neural Networks

Purpose: We developed a method to automatically locate and quantify graft detachment after Descemets Membrane Endothelial Keratoplasty (DMEK) in Anterior Segment Optical Coherence Tomography (AS-OCT) scans. Methods: 1280 AS-OCT B-scans were annotated by a DMEK expert. Using the annotations, a deep learning pipeline was developed to localize scleral spur, center the AS-OCT B-scans and segment the detached graft sections. Detachment segmentation model performance was evaluated per B-scan by comparing (1) length of detachment and (2) horizontal projection of the detached sections with the expert annotations. Horizontal projections were used to construct graft detachment maps. All final evaluations were done on a test set that was set apart during training of the models. A second DMEK expert annotated the test set to determine inter-rater performance. Results: Mean scleral spur localization error was 0.155 mm, whereas the inter-rater difference was 0.090 mm. The estimated graft detachment lengths were in 69% of the cases within a 10-pixel (~150{mu}m) difference from the ground truth (77% for the second DMEK expert). Dice scores for the horizontal projections of all B-scans with detachments were 0.896 and 0.880 for our model and the second DMEK expert respectively. Conclusion: Our deep learning model can be used to automatically and instantly localize graft detachment in AS-OCT B-scans. Horizontal detachment projections can be determined with the same accuracy as a human DMEK expert, allowing for the construction of accurate graft detachment maps. Translational Relevance: Automated localization and quantification of graft detachment can support DMEK research and standardize clinical decision making.

Corneal thickness (pachymetry) maps can be used to monitor restoration of corneal endothelial function, for example after Descemets membrane endothelial keratoplasty (DMEK). Automated delineation of the corneal interfaces in anterior segment optical coherence tomography (AS-OCT) can be challenging for corneas that are irregularly shaped due to pathology, or as a consequence of surgery, leading to incorrect thickness measurements. In this research, deep learning is used to automatically delineate the corneal interfaces and measure corneal thickness with high accuracy in post-DMEK AS-OCT B-scans. Three different deep learning strategies were developed based on 960 B-scans from 50 patients. On an independent test set of 320 B-scans, corneal thickness could be measured with an error of 13.98 to 15.50 micrometer for the central 9 mm range, which is less than 3% of the average corneal thickness. The accurate thickness measurements were used to construct detailed pachymetry maps. Moreover, follow-up scans could be registered based on anatomical landmarks to obtain differential pachymetry maps. These maps may enable a more comprehensive understanding of the restoration of the endothelial function after DMEK, where thickness often varies throughout different regions of the cornea, and subsequently contribute to a standardized postoperative regime.
Many real-world signal sources are complex-valued, having real and imaginary components. However, the vast majority of existing deep learning platforms and network architectures do not support the use of complex-valued data. MRI data is inherently complex-valued, so existing approaches discard the richer algebraic structure of the complex data. In this work, we investigate end-to-end complex-valued convolutional neural networks - specifically, for image reconstruction in lieu of two-channel real-valued networks. We apply this to magnetic resonance imaging reconstruction for the purpose of accelerating scan times and determine the performance of various promising complex-valued activation functions. We find that complex-valued CNNs with complex-valued convolutions provide superior reconstructions compared to real-valued convolutions with the same number of trainable parameters, over a variety of network architectures and datasets.
Hyperspectral images are of crucial importance in order to better understand features of different materials. To reach this goal, they leverage on a high number of spectral bands. However, this interesting characteristic is often paid by a reduced spatial resolution compared with traditional multispectral image systems. In order to alleviate this issue, in this work, we propose a simple and efficient architecture for deep convolutional neural networks to fuse a low-resolution hyperspectral image (LR-HSI) and a high-resolution multispectral image (HR-MSI), yielding a high-resolution hyperspectral image (HR-HSI). The network is designed to preserve both spatial and spectral information thanks to an architecture from two folds: one is to utilize the HR-HSI at a different scale to get an output with a satisfied spectral preservation; another one is to apply concepts of multi-resolution analysis to extract high-frequency information, aiming to output high quality spatial details. Finally, a plain mean squared error loss function is used to measure the performance during the training. Extensive experiments demonstrate that the proposed network architecture achieves best performance (both qualitatively and quantitatively) compared with recent state-of-the-art hyperspectral image super-resolution approaches. Moreover, other significant advantages can be pointed out by the use of the proposed approach, such as, a better network generalization ability, a limited computational burden, and a robustness with respect to the number of training samples.
Colorectal cancer is the third most common cancer-related death after lung cancer and breast cancer worldwide. The risk of developing colorectal cancer could be reduced by early diagnosis of polyps during a colonoscopy. Computer-aided diagnosis systems have the potential to be applied for polyp screening and reduce the number of missing polyps. In this paper, we compare the performance of different deep learning architectures as feature extractors, i.e. ResNet, DenseNet, InceptionV3, InceptionResNetV2 and SE-ResNeXt in the encoder part of a U-Net architecture. We validated the performance of presented ensemble models on the CVC-Clinic (GIANA 2018) dataset. The DenseNet169 feature extractor combined with U-Net architecture outperformed the other counterparts and achieved an accuracy of 99.15%, Dice similarity coefficient of 90.87%, and Jaccard index of 83.82%.
Knee osteoarthritis (OA) is the most common musculoskeletal disease in the world. In primary healthcare, knee OA is diagnosed using clinical examination and radiographic assessment. Osteoarthritis Research Society International (OARSI) atlas of OA radiographic features allows to perform independent assessment of knee osteophytes, joint space narrowing and other knee features. This provides a fine-grained OA severity assessment of the knee, compared to the gold standard and most commonly used Kellgren-Lawrence (KL) composite score. However, both OARSI and KL grading systems suffer from moderate inter-rater agreement, and therefore, the use of computer-aided methods could help to improve the reliability of the process. In this study, we developed a robust, automatic method to simultaneously predict KL and OARSI grades in knee radiographs. Our method is based on Deep Learning and leverages an ensemble of deep residual networks with 50 layers, squeeze-excitation and ResNeXt blocks. Here, we used transfer learning from ImageNet with a fine-tuning on the whole Osteoarthritis Initiative (OAI) dataset. An independent testing of our model was performed on the whole Multicenter Osteoarthritis Study (MOST) dataset. Our multi-task method yielded Cohens kappa coefficients of 0.82 for KL-grade and 0.79, 0.84, 0.94, 0.83, 0.84, 0.90 for femoral osteophytes, tibial osteophytes and joint space narrowing for lateral and medial compartments respectively. Furthermore, our method yielded area under the ROC curve of 0.98 and average precision of 0.98 for detecting the presence of radiographic OA (KL $geq 2$), which is better than the current state-of-the-art.
