Robust Brain Magnetic Resonance Image Segmentation for Hydrocephalus Patients: Hard and Soft Attention

Brain magnetic resonance (MR) segmentation for hydrocephalus patients is considered as a challenging work. Encoding the variation of the brain anatomical structures from different individuals cannot be easily achieved. The task becomes even more difficult especially when the image data from hydrocephalus patients are considered, which often have large deformations and differ significantly from the normal subjects. Here, we propose a novel strategy with hard and soft attention modules to solve the segmentation problems for hydrocephalus MR images. Our main contributions are three-fold: 1) the hard-attention module generates coarse segmentation map using multi-atlas-based method and the VoxelMorph tool, which guides subsequent segmentation process and improves its robustness; 2) the soft-attention module incorporates position attention to capture precise context information, which further improves the segmentation accuracy; 3) we validate our method by segmenting insula, thalamus and many other regions-of-interests (ROIs) that are critical to quantify brain MR images of hydrocephalus patients in real clinical scenario. The proposed method achieves much improved robustness and accuracy when segmenting all 17 consciousness-related ROIs with high variations for different subjects. To the best of our knowledge, this is the first work to employ deep learning for solving the brain segmentation problems of hydrocephalus patients.

Whole brain extraction, also known as skull stripping, is a process in neuroimaging in which non-brain tissue such as skull, eyeballs, skin, etc. are removed from neuroimages. Skull striping is a preliminary step in presurgical planning, cortical rec onstruction, and automatic tumor segmentation. Despite a plethora of skull stripping approaches in the literature, few are sufficiently accurate for processing pathology-presenting MRIs, especially MRIs with brain tumors. In this work we propose a deep learning approach for skull striping common MRI sequences in oncology such as T1-weighted with gadolinium contrast (T1Gd) and T2-weighted fluid attenuated inversion recovery (FLAIR) in patients with brain tumors. We automatically created gray matter, white matter, and CSF probability masks using SPM12 software and merged the masks into one for a final whole-brain mask for model training. Dice agreement, sensitivity, and specificity of the model (referred herein as DeepBrain) was tested against manual brain masks. To assess data efficiency, we retrained our models using progressively fewer training data examples and calculated average dice scores on the test set for the models trained in each round. Further, we tested our model against MRI of healthy brains from the LBP40A dataset. Overall, DeepBrain yielded an average dice score of 94.5%, sensitivity of 96.4%, and specificity of 98.5% on brain tumor data. For healthy brains, model performance improved to a dice score of 96.2%, sensitivity of 96.6% and specificity of 99.2%. The data efficiency experiment showed that, for this specific task, comparable levels of accuracy could have been achieved with as few as 50 training samples. In conclusion, this study demonstrated that a deep learning model trained on minimally processed automatically-generated labels can generate more accurate brain masks on MRI of brain tumor patients within seconds.
Learning structural information is critical for producing an ideal result in retinal image segmentation. Recently, convolutional neural networks have shown a powerful ability to extract effective representations. However, convolutional and pooling op erations filter out some useful structural information. In this paper, we propose an Attention Guided Network (AG-Net) to preserve the structural information and guide the expanding operation. In our AG-Net, the guided filter is exploited as a structure sensitive expanding path to transfer structural information from previous feature maps, and an attention block is introduced to exclude the noise and reduce the negative influence of background further. The extensive experiments on two retinal image segmentation tasks (i.e., blood vessel segmentation, optic disc and cup segmentation) demonstrate the effectiveness of our proposed method.
Normal Pressure Hydrocephalus (NPH) is one of the few reversible forms of dementia, Due to their low cost and versatility, Computed Tomography (CT) scans have long been used as an aid to help diagnose intracerebral anomalies such as NPH. However, no well-defined and effective protocol currently exists for the analysis of CT scan-based ventricular, cerebral mass and subarachnoid space volumes in the setting of NPH. The Evans ratio, an approximation of the ratio of ventricle to brain volume using only one 2D slice of the scan, has been proposed but is not robust. Instead of manually measuring a 2-dimensional proxy for the ratio of ventricle volume to brain volume, this study proposes an automated method of calculating the brain volumes for better recognition of NPH from a radiological standpoint. The method first aligns the subject CT volume to a common space through an affine transformation, then uses a random forest classifier to mask relevant tissue types. A 3D morphological segmentation method is used to partition the brain volume, which in turn is used to train machine learning methods to classify the subjects into non-NPH vs. NPH based on volumetric information. The proposed algorithm has increased sensitivity compared to the Evans ratio thresholding method.
Brain tumor is the most common and deadliest disease that can be found in all age groups. Generally, MRI modality is adopted for identifying and diagnosing tumors by the radiologists. The correct identification of tumor regions and its type can aid t o diagnose tumors with the followup treatment plans. However, for any radiologist analysing such scans is a complex and time-consuming task. Motivated by the deep learning based computer-aided-diagnosis systems, this paper proposes multi-task attention guided encoder-decoder network (MAG-Net) to classify and segment the brain tumor regions using MRI images. The MAG-Net is trained and evaluated on the Figshare dataset that includes coronal, axial, and sagittal views with 3 types of tumors meningioma, glioma, and pituitary tumor. With exhaustive experimental trials the model achieved promising results as compared to existing state-of-the-art models, while having least number of training parameters among other state-of-the-art models.

The performance of deep segmentation models often degrades due to distribution shifts in image intensities between the training and test data sets. This is particularly pronounced in multi-centre studies involving data acquired using multi-vendor sca nners, with variations in acquisition protocols. It is challenging to address this degradation because the shift is often not known textit{a priori} and hence difficult to model. We propose a novel framework to ensure robust segmentation in the presence of such distribution shifts. Our contribution is three-fold. First, inspired by the spirit of curriculum learning, we design a novel style curriculum to train the segmentation models using an easy-to-hard mode. A style transfer model with style fusion is employed to generate the curriculum samples. Gradually focusing on complex and adversarial style samples can significantly boost the robustness of the models. Second, instead of subjectively defining the curriculum complexity, we adopt an automated gradient manipulation method to control the hard and adversarial sample generation process. Third, we propose the Local Gradient Sign strategy to aggregate the gradient locally and stabilise training during gradient manipulation. The proposed framework can generalise to unknown distribution without using any target data. Extensive experiments on the public M&Ms Challenge dataset demonstrate that our proposed framework can generalise deep models well to unknown distributions and achieve significant improvements in segmentation accuracy.
