We present the first system that provides real-time probe movement guidance for acquiring standard planes in routine freehand obstetric ultrasound scanning. Such a system can contribute to the worldwide deployment of obstetric ultrasound scanning by lowering the required level of operator expertise. The system employs an artificial neural network that receives the ultrasound video signal and the motion signal of an inertial measurement unit (IMU) that is attached to the probe, and predicts a guidance signal. The network termed US-GuideNet predicts either the movement towards the standard plane position (goal prediction), or the next movement that an expert sonographer would perform (action prediction). While existing models for other ultrasound applications are trained with simulations or phantoms, we train our model with real-world ultrasound video and probe motion data from 464 routine clinical scans by 17 accredited sonographers. Evaluations for 3 standard plane types show that the model provides a useful guidance signal with an accuracy of 88.8% for goal prediction and 90.9% for action prediction.

Automatic breast lesion segmentation in ultrasound helps to diagnose breast cancer, which is one of the dreadful diseases that affect women globally. Segmenting breast regions accurately from ultrasound image is a challenging task due to the inherent speckle artifacts, blurry breast lesion boundaries, and inhomogeneous intensity distributions inside the breast lesion regions. Recently, convolutional neural networks (CNNs) have demonstrated remarkable results in medical image segmentation tasks. However, the convolutional operations in a CNN often focus on local regions, which suffer from limited capabilities in capturing long-range dependencies of the input ultrasound image, resulting in degraded breast lesion segmentation accuracy. In this paper, we develop a deep convolutional neural network equipped with a global guidance block (GGB) and breast lesion boundary detection (BD) modules for boosting the breast ultrasound lesion segmentation. The GGB utilizes the multi-layer integrated feature map as a guidance information to learn the long-range non-local dependencies from both spatial and channel domains. The BD modules learn additional breast lesion boundary map to enhance the boundary quality of a segmentation result refinement. Experimental results on a public dataset and a collected dataset show that our network outperforms other medical image segmentation methods and the recent semantic segmentation methods on breast ultrasound lesion segmentation. Moreover, we also show the application of our network on the ultrasound prostate segmentation, in which our method better identifies prostate regions than state-of-the-art networks.
Lung ultrasound (LUS) imaging is used to assess lung abnormalities, including the presence of B-line artefacts due to fluid leakage into the lungs caused by a variety of diseases. However, manual detection of these artefacts is challenging. In this paper, we propose a novel methodology to automatically detect and localize B-lines in LUS videos using deep neural networks trained with weak labels. To this end, we combine a convolutional neural network (CNN) with a long short-term memory (LSTM) network and a temporal attention mechanism. Four different models are compared using data from 60 patients. Results show that our best model can determine whether one-second clips contain B-lines or not with an F1 score of 0.81, and extracts a representative frame with B-lines with an accuracy of 87.5%.
Ultrasound (US) image segmentation embraced its significant improvement in deep learning era. However, the lack of sharp boundaries in US images still remains an inherent challenge for segmentation. Previous methods often resort to global context, multi-scale cues or auxiliary guidance to estimate the boundaries. It is hard for these methods to approach pixel-level learning for fine-grained boundary generating. In this paper, we propose a novel and effective framework to improve boundary estimation in US images. Our work has three highlights. First, we propose to formulate the boundary estimation as a rendering task, which can recognize ambiguous points (pixels/voxels) and calibrate the boundary prediction via enriched feature representation learning. Second, we introduce point-wise contrastive learning to enhance the similarity of points from the same class and contrastively decrease the similarity of points from different classes. Boundary ambiguities are therefore further addressed. Third, both rendering and contrastive learning tasks contribute to consistent improvement while reducing network parameters. As a proof-of-concept, we performed validation experiments on a challenging dataset of 86 ovarian US volumes. Results show that our proposed method outperforms state-of-the-art methods and has the potential to be used in clinical practice.

Background and Objective: Biometric measurements of fetal head are important indicators for maternal and fetal health monitoring during pregnancy. 3D ultrasound (US) has unique advantages over 2D scan in covering the whole fetal head and may promote the diagnoses. However, automatically segmenting the whole fetal head in US volumes still pends as an emerging and unsolved problem. The challenges that automated solutions need to tackle include the poor image quality, boundary ambiguity, long-span occlusion, and the appearance variability across different fetal poses and gestational ages. In this paper, we propose the first fully-automated solution to segment the whole fetal head in US volumes. Methods: The segmentation task is firstly formulated as an end-to-end volumetric mapping under an encoder-decoder deep architecture. We then combine the segmentor with a proposed hybrid attention scheme (HAS) to select discriminative features and suppress the non-informative volumetric features in a composite and hierarchical way. With little computation overhead, HAS proves to be effective in addressing boundary ambiguity and deficiency. To enhance the spatial consistency in segmentation, we further organize multiple segmentors in a cascaded fashion to refine the results by revisiting context in the prediction of predecessors. Results: Validated on a large dataset collected from 100 healthy volunteers, our method presents superior segmentation performance (DSC (Dice Similarity Coefficient), 96.05%), remarkable agreements with experts. With another 156 volumes collected from 52 volunteers, we ahieve high reproducibilities (mean standard deviation 11.524 mL) against scan variations. Conclusion: This is the first investigation about whole fetal head segmentation in 3D US. Our method is promising to be a feasible solution in assisting the volumetric US-based prenatal studies.
Fine-tuning a network which has been trained on a large dataset is an alternative to full training in order to overcome the problem of scarce and expensive data in medical applications. While the shallow layers of the network are usually kept unchanged, deeper layers are modified according to the new dataset. This approach may not work for ultrasound images due to their drastically different appearance. In this study, we investigated the effect of fine-tuning different layers of a U-Net which was trained on segmentation of natural images in breast ultrasound image segmentation. Tuning the contracting part and fixing the expanding part resulted in substantially better results compared to fixing the contracting part and tuning the expanding part. Furthermore, we showed that starting to fine-tune the U-Net from the shallow layers and gradually including more layers will lead to a better performance compared to fine-tuning the network from the deep layers moving back to shallow layers. We did not observe the same results on segmentation of X-ray images, which have different salient features compared to ultrasound, it may therefore be more appropriate to fine-tune the shallow layers rather than deep layers. Shallow layers learn lower level features (including speckle pattern, and probably the noise and artifact properties) which are critical in automatic segmentation in this modality.
