Background:The electrocardiogram (ECG) is one of the most commonly used diagnostic tools in medicine and healthcare. Deep learning methods have achieved promising results on predictive healthcare tasks using ECG signals. Objective:This paper presents a systematic review of deep learning methods for ECG data from both modeling and application perspectives. Methods:We extracted papers that applied deep learning (deep neural network) models to ECG data that were published between Jan. 1st of 2010 and Feb. 29th of 2020 from Google Scholar, PubMed, and the DBLP. We then analyzed each article according to three factors: tasks, models, and data. Finally, we discuss open challenges and unsolved problems in this area. Results: The total number of papers extracted was 191. Among these papers, 108 were published after 2019. Different deep learning architectures have been used in various ECG analytics tasks, such as disease detection/classification, annotation/localization, sleep staging, biometric human identification, and denoising. Conclusion: The number of works on deep learning for ECG data has grown explosively in recent years. Such works have achieved accuracy comparable to that of traditional feature-based approaches and ensembles of multiple approaches can achieve even better results. Specifically, we found that a hybrid architecture of a convolutional neural network and recurrent neural network ensemble using expert features yields the best results. However, there are some new challenges and problems related to interpretability, scalability, and efficiency that must be addressed. Furthermore, it is also worth investigating new applications from the perspectives of datasets and methods. Significance: This paper summarizes existing deep learning research using ECG data from multiple perspectives and highlights existing challenges and problems to identify potential future research directions.

Objective: Temporal electronic health records (EHRs) can be a wealth of information for secondary uses, such as clinical events prediction or chronic disease management. However, challenges exist for temporal data representation. We therefore sought to identify these challenges and evaluate novel methodologies for addressing them through a systematic examination of deep learning solutions. Methods: We searched five databases (PubMed, EMBASE, the Institute of Electrical and Electronics Engineers [IEEE] Xplore Digital Library, the Association for Computing Machinery [ACM] digital library, and Web of Science) complemented with hand-searching in several prestigious computer science conference proceedings. We sought articles that reported deep learning methodologies on temporal data representation in structured EHR data from January 1, 2010, to August 30, 2020. We summarized and analyzed the selected articles from three perspectives: nature of time series, methodology, and model implementation. Results: We included 98 articles related to temporal data representation using deep learning. Four major challenges were identified, including data irregularity, data heterogeneity, data sparsity, and model opacity. We then studied how deep learning techniques were applied to address these challenges. Finally, we discuss some open challenges arising from deep learning. Conclusion: Temporal EHR data present several major challenges for clinical prediction modeling and data utilization. To some extent, current deep learning solutions can address these challenges. Future studies can consider designing comprehensive and integrated solutions. Moreover, researchers should incorporate additional clinical domain knowledge into study designs and enhance the interpretability of the model to facilitate its implementation in clinical practice.
Cardiovascular disorders account for nearly 1 in 3 deaths in the United States. Care for these disorders are often determined during visits to acute care facilities, such as hospitals. While the length of stay in these settings represents just a small proportion of patients lives, they account for a disproportionately large amount of decision making. To overcome this bias towards data from acute care settings, there is a need for longitudinal monitoring in patients with cardiovascular disorders. Longitudinal monitoring can provide a more comprehensive picture of patient health, allowing for more informed decision making. This work surveys the current field of sensing technologies and machine learning analytics that exist in the field of remote monitoring for cardiovascular disorders. We highlight three primary needs in the design of new smart health technologies: 1) the need for sensing technology that can track longitudinal trends in signs and symptoms of the cardiovascular disorder despite potentially infrequent, noisy, or missing data measurements; 2) the need for new analytic techniques that model data captured in a longitudinal, continual fashion to aid in the development of new risk prediction techniques and in tracking disease progression; and 3) the need for machine learning techniques that are personalized and interpretable, allowing for advancements in shared clinical decision making. We highlight these needs based upon the current state-of-the-art in smart health technologies and analytics and discuss the ample opportunities that exist in addressing all three needs in the development of smart health technologies and analytics applied to the field of cardiovascular disorders and care.
Electrocardiogram (ECG) is one of the most convenient and non-invasive tools for monitoring peoples heart condition, which can use for diagnosing a wide range of heart diseases, including Cardiac Arrhythmia, Acute Coronary Syndrome, et al. However, traditional ECG disease detection models show substantial rates of misdiagnosis due to the limitations of the abilities of extracted features. Recent deep learning methods have shown significant advantages, but they do not provide publicly available services for those who have no training data or computational resources. In this paper, we demonstrate our work on building, training, and serving such out-of-the-box cloud deep learning service for cardiac disease detection from ECG named CardioLearn. The analytic ability of any other ECG recording devices can be enhanced by connecting to the Internet and invoke our open API. As a practical example, we also design a portable smart hardware device along with an interactive mobile program, which can collect ECG and detect potential cardiac diseases anytime and anywhere.
The astounding success made by artificial intelligence (AI) in healthcare and other fields proves that AI can achieve human-like performance. However, success always comes with challenges. Deep learning algorithms are data-dependent and require large datasets for training. The lack of data in the medical imaging field creates a bottleneck for the application of deep learning to medical image analysis. Medical image acquisition, annotation, and analysis are costly, and their usage is constrained by ethical restrictions. They also require many resources, such as human expertise and funding. That makes it difficult for non-medical researchers to have access to useful and large medical data. Thus, as comprehensive as possible, this paper provides a collection of medical image datasets with their associated challenges for deep learning research. We have collected information of around three hundred datasets and challenges mainly reported between 2013 and 2020 and categorized them into four categories: head & neck, chest & abdomen, pathology & blood, and ``others. Our paper has three purposes: 1) to provide a most up to date and complete list that can be used as a universal reference to easily find the datasets for clinical image analysis, 2) to guide researchers on the methodology to test and evaluate their methods performance and robustness on relevant datasets, 3) to provide a ``route to relevant algorithms for the relevant medical topics, and challenge leaderboards.
Respiratory ailments afflict a wide range of people and manifests itself through conditions like asthma and sleep apnea. Continuous monitoring of chronic respiratory ailments is seldom used outside the intensive care ward due to the large size and cost of the monitoring system. While Electrocardiogram (ECG) based respiration extraction is a validated approach, its adoption is limited by access to a suitable continuous ECG monitor. Recently, due to the widespread adoption of wearable smartwatches with in-built Photoplethysmogram (PPG) sensor, it is being considered as a viable candidate for continuous and unobtrusive respiration monitoring. Research in this domain, however, has been predominantly focussed on estimating respiration rate from PPG. In this work, a novel end-to-end deep learning network called RespNet is proposed to perform the task of extracting the respiration signal from a given input PPG as opposed to extracting respiration rate. The proposed network was trained and tested on two different datasets utilizing different modalities of reference respiration signal recordings. Also, the similarity and performance of the proposed network against two conventional signal processing approaches for extracting respiration signal were studied. The proposed method was tested on two independent datasets with a Mean Squared Error of 0.262 and 0.145. The Cross-Correlation coefficient of the respective datasets were found to be 0.933 and 0.931. The reported errors and similarity was found to be better than conventional approaches. The proposed approach would aid clinicians to provide comprehensive evaluation of sleep-related respiratory conditions and chronic respiratory ailments while being comfortable and inexpensive for the patient.
