Radiation therapy presents a need for dynamic tracking of a target tumor volume. Fiducial markers such as implanted gold seeds have been used to gate radiation delivery but the markers are invasive and gating significantly increases treatment time. Pretreatment acquisition of a respiratory correlated 4DCT allows for determination of accurate motion tracking which is useful in treatment planning. We design a patient-specific motion subspace and a deep convolutional neural network to recover anatomical positions from a single fluoroscopic projection in real-time. We use this deep network to approximate the nonlinear inverse of a diffeomorphic deformation composed with radiographic projection. This network recovers subspace coordinates to define the patient-specific deformation of the lungs from a baseline anatomic position. The geometric accuracy of the subspace deformations on real patient data is similar to accuracy attained by original image registration between individual respiratory-phase image volumes.

Image registration and in particular deformable registration methods are pillars of medical imaging. Inspired by the recent advances in deep learning, we propose in this paper, a novel convolutional neural network architecture that couples linear and deformable registration within a unified architecture endowed with near real-time performance. Our framework is modular with respect to the global transformation component, as well as with respect to the similarity function while it guarantees smooth displacement fields. We evaluate the performance of our network on the challenging problem of MRI lung registration, and demonstrate superior performance with respect to state of the art elastic registration methods. The proposed deformation (between inspiration & expiration) was considered within a clinically relevant task of interstitial lung disease (ILD) classification and showed promising results.
Spatially aligning medical images from different modalities remains a challenging task, especially for intraoperative applications that require fast and robust algorithms. We propose a weakly-supervised, label-driven formulation for learning 3D voxel correspondence from higher-level label correspondence, thereby bypassing classical intensity-based image similarity measures. During training, a convolutional neural network is optimised by outputting a dense displacement field (DDF) that warps a set of available anatomical labels from the moving image to match their corresponding counterparts in the fixed image. These label pairs, including solid organs, ducts, vessels, point landmarks and other ad hoc structures, are only required at training time and can be spatially aligned by minimising a cross-entropy function of the warped moving label and the fixed label. During inference, the trained network takes a new image pair to predict an optimal DDF, resulting in a fully-automatic, label-free, real-time and deformable registration. For interventional applications where large global transformation prevails, we also propose a neural network architecture to jointly optimise the global- and local displacements. Experiment results are presented based on cross-validating registrations of 111 pairs of T2-weighted magnetic resonance images and 3D transrectal ultrasound images from prostate cancer patients with a total of over 4000 anatomical labels, yielding a median target registration error of 4.2 mm on landmark centroids and a median Dice of 0.88 on prostate glands.

Deep learning-based point cloud registration models are often generalized from extensive training over a large volume of data to learn the ability to predict the desired geometric transformation to register 3D point clouds. In this paper, we propose a meta-learning based 3D registration model, named 3D Meta-Registration, that is capable of rapidly adapting and well generalizing to new 3D registration tasks for unseen 3D point clouds. Our 3D Meta-Registration gains a competitive advantage by training over a variety of 3D registration tasks, which leads to an optimized model for the best performance on the distribution of registration tasks including potentially unseen tasks. Specifically, the proposed 3D Meta-Registration model consists of two modules: 3D registration learner and 3D registration meta-learner. During the training, the 3D registration learner is trained to complete a specific registration task aiming to determine the desired geometric transformation that aligns the source point cloud with the target one. In the meantime, the 3D registration meta-learner is trained to provide the optimal parameters to update the 3D registration learner based on the learned task distribution. After training, the 3D registration meta-learner, which is learned with the optimized coverage of distribution of 3D registration tasks, is able to dynamically update 3D registration learners with desired parameters to rapidly adapt to new registration tasks. We tested our model on synthesized dataset ModelNet and FlyingThings3D, as well as real-world dataset KITTI. Experimental results demonstrate that 3D Meta-Registration achieves superior performance over other previous techniques (e.g. FlowNet3D).
Differentiable rendering is a technique to connect 3D scenes with corresponding 2D images. Since it is differentiable, processes during image formation can be learned. Previous approaches to differentiable rendering focus on mesh-based representations of 3D scenes, which is inappropriate for medical applications where volumetric, voxelized models are used to represent anatomy. We propose a novel Projective Spatial Transformer module that generalizes spatial transformers to projective geometry, thus enabling differentiable volume rendering. We demonstrate the usefulness of this architecture on the example of 2D/3D registration between radiographs and CT scans. Specifically, we show that our transformer enables end-to-end learning of an image processing and projection model that approximates an image similarity function that is convex with respect to the pose parameters, and can thus be optimized effectively using conventional gradient descent. To the best of our knowledge, this is the first time that spatial transformers have been described for projective geometry. The source code will be made public upon publication of this manuscript and we hope that our developments will benefit related 3D research applications.
This study investigates the use of the unsupervised deep learning framework VoxelMorph for deformable registration of longitudinal abdominopelvic CT images acquired in patients with bone metastases from breast cancer. The CT images were refined prior to registration by automatically removing the CT table and all other extra-corporeal components. To improve the learning capabilities of VoxelMorph when only a limited amount of training data is available, a novel incremental training strategy is proposed based on simulated deformations of consecutive CT images. In a 4-fold cross-validation scheme, the incremental training strategy achieved significantly better registration performance compared to training on a single volume. Although our deformable image registration method did not outperform iterative registration using NiftyReg (considered as a benchmark) in terms of registration quality, the registrations were approximately 300 times faster. This study showed the feasibility of deep learning based deformable registration of longitudinal abdominopelvic CT images via a novel incremental training strategy based on simulated deformations.
