Automatic Ischemic Stroke Lesion Segmentation from Computed Tomography Perfusion Images by Image Synthesis and Attention-Based Deep Neural Networks

Ischemic stroke lesion segmentation from Computed Tomography Perfusion (CTP) images is important for accurate diagnosis of stroke in acute care units. However, it is challenged by low image contrast and resolution of the perfusion parameter maps, in addition to the complex appearance of the lesion. To deal with this problem, we propose a novel framework based on synthesized pseudo Diffusion-Weighted Imaging (DWI) from perfusion parameter maps to obtain better image quality for more accurate segmentation. Our framework consists of three components based on Convolutional Neural Networks (CNNs) and is trained end-to-end. First, a feature extractor is used to obtain both a low-level and high-level compact representation of the raw spatiotemporal Computed Tomography Angiography (CTA) images. Second, a pseudo DWI generator takes as input the concatenation of CTP perfusion parameter maps and our extracted features to obtain the synthesized pseudo DWI. To achieve better synthesis quality, we propose a hybrid loss function that pays more attention to lesion regions and encourages high-level contextual consistency. Finally, we segment the lesion region from the synthesized pseudo DWI, where the segmentation network is based on switchable normalization and channel calibration for better performance. Experimental results showed that our framework achieved the top performance on ISLES 2018 challenge and: 1) our method using synthesized pseudo DWI outperformed methods segmenting the lesion from perfusion parameter maps directly; 2) the feature extractor exploiting additional spatiotemporal CTA images led to better synthesized pseudo DWI quality and higher segmentation accuracy; and 3) the proposed loss functions and network structure improved the pseudo DWI synthesis and lesion segmentation performance.

Recently, accurate mandible segmentation in CT scans based on deep learning methods has attracted much attention. However, there still exist two major challenges, namely, metal artifacts among mandibles and large variations in shape or size among ind ividuals. To address these two challenges, we propose a recurrent segmentation convolutional neural network (RSegCNN) that embeds segmentation convolutional neural network (SegCNN) into the recurrent neural network (RNN) for robust and accurate segmentation of the mandible. Such a design of the system takes into account the similarity and continuity of the mandible shapes captured in adjacent image slices in CT scans. The RSegCNN infers the mandible information based on the recurrent structure with the embedded encoder-decoder segmentation (SegCNN) components. The recurrent structure guides the system to exploit relevant and important information from adjacent slices, while the SegCNN component focuses on the mandible shapes from a single CT slice. We conducted extensive experiments to evaluate the proposed RSegCNN on two head and neck CT datasets. The experimental results show that the RSegCNN is significantly better than the state-of-the-art models for accurate mandible segmentation.
Diffusion-weighted (DW) magnetic resonance imaging is essential for the diagnosis and treatment of ischemic stroke. DW images (DWIs) are usually acquired in multi-slice settings where lesion areas in two consecutive 2D slices are highly discontinuous due to large slice thickness and sometimes even slice gaps. Therefore, although DWIs contain rich 3D information, they cannot be treated as regular 3D or 2D images. Instead, DWIs are somewhere in-between (or 2.5D) due to the volumetric nature but inter-slice discontinuities. Thus, it is not ideal to apply most existing segmentation methods as they are designed for either 2D or 3D images. To tackle this problem, we propose a new neural network architecture tailored for segmenting highly-discontinuous 2.5D data such as DWIs. Our network, termed LambdaUNet, extends UNet by replacing convolutional layers with our proposed Lambda+ layers. In particular, Lambda+ layers transform both intra-slice and inter-slice context around a pixel into linear functions, called lambdas, which are then applied to the pixel to produce informative 2.5D features. LambdaUNet is simple yet effective in combining sparse inter-slice information from adjacent slices while also capturing dense contextual features within a single slice. Experiments on a unique clinical dataset demonstrate that LambdaUNet outperforms existing 3D/2D image segmentation methods including recent variants of UNet. Code for LambdaUNet will be released with the publication to facilitate future research.
Type-B Aortic Dissection (TBAD) is one of the most serious cardiovascular events characterized by a growing yearly incidence,and the severity of disease prognosis. Currently, computed tomography angiography (CTA) has been widely adopted for the diagn osis and prognosis of TBAD. Accurate segmentation of true lumen (TL), false lumen (FL), and false lumen thrombus (FLT) in CTA are crucial for the precise quantification of anatomical features. However, existing works only focus on only TL and FL without considering FLT. In this paper, we propose ImageTBAD, the first 3D computed tomography angiography (CTA) image dataset of TBAD with annotation of TL, FL, and FLT. The proposed dataset contains 100 TBAD CTA images, which is of decent size compared with existing medical imaging datasets. As FLT can appear almost anywhere along the aorta with irregular shapes, segmentation of FLT presents a wide class of segmentation problems where targets exist in a variety of positions with irregular shapes. We further propose a baseline method for automatic segmentation of TBAD. Results show that the baseline method can achieve comparable results with existing works on aorta and TL segmentation. However, the segmentation accuracy of FLT is only 52%, which leaves large room for improvement and also shows the challenge of our dataset. To facilitate further research on this challenging problem, our dataset and codes are released to the public.
The insufficiency of annotated medical imaging scans for cancer makes it challenging to train and validate data-hungry deep learning models in precision oncology. We propose a new richer generative adversarial network for free-form 3D tumor/lesion sy nthesis in computed tomography (CT) images. The network is composed of a new richer convolutional feature enhanced dilated-gated generator (RicherDG) and a hybrid loss function. The RicherDG has dilated-gated convolution layers to enable tumor-painting and to enlarge perceptive fields; and it has a novel richer convolutional feature association branch to recover multi-scale convolutional features especially from uncertain boundaries between tumor and surrounding healthy tissues. The hybrid loss function, which consists of a diverse range of losses, is designed to aggregate complementary information to improve optimization. We perform a comprehensive evaluation of the synthesis results on a wide range of public CT image datasets covering the liver, kidney tumors, and lung nodules. The qualitative and quantitative evaluations and ablation study demonstrated improved synthesizing results over advanced tumor synthesis methods.
Segmenting stroke lesions from T1-weighted MR images is of great value for large-scale stroke rehabilitation neuroimaging analyses. Nevertheless, there are great challenges with this task, such as large range of stroke lesion scales and the tissue in tensity similarity. The famous encoder-decoder convolutional neural network, which although has made great achievements in medical image segmentation areas, may fail to address these challenges due to the insufficient uses of multi-scale features and context information. To address these challenges, this paper proposes a Cross-Level fusion and Context Inference Network (CLCI-Net) for the chronic stroke lesion segmentation from T1-weighted MR images. Specifically, a Cross-Level feature Fusion (CLF) strategy was developed to make full use of different scale features across different levels; Extending Atrous Spatial Pyramid Pooling (ASPP) with CLF, we have enriched multi-scale features to handle the different lesion sizes; In addition, convolutional long short-term memory (ConvLSTM) is employed to infer context information and thus capture fine structures to address the intensity similarity issue. The proposed approach was evaluated on an open-source dataset, the Anatomical Tracings of Lesions After Stroke (ATLAS) with the results showing that our network outperforms five state-of-the-art methods.
