A Transfer Learning Approach for Automated Segmentation of Prostate Whole Gland and Transition Zone in Diffusion Weighted MRI

The segmentation of prostate whole gland and transition zone in Diffusion Weighted MRI (DWI) are the first step in designing computer-aided detection algorithms for prostate cancer. However, variations in MRI acquisition parameters and scanner manufacturing result in different appearances of prostate tissue in the images. Convolutional neural networks (CNNs) which have shown to be successful in various medical image analysis tasks including segmentation are typically sensitive to the variations in imaging parameters. This sensitivity leads to poor segmentation performance of CNNs trained on a source cohort and tested on a target cohort from a different scanner and hence, it limits the applicability of CNNs for cross-cohort training and testing. Contouring prostate whole gland and transition zone in DWI images are time-consuming and expensive. Thus, it is important to enable CNNs pretrained on images of source domain, to segment images of target domain with minimum requirement for manual segmentation of images from the target domain. In this work, we propose a transfer learning method based on a modified U-net architecture and loss function, for segmentation of prostate whole gland and transition zone in DWIs using a CNN pretrained on a source dataset and tested on the target dataset. We explore the effect of the size of subset of target dataset used for fine-tuning the pre-trained CNN on the overall segmentation accuracy. Our results show that with a fine-tuning data as few as 30 patients from the target domain, the proposed transfer learning-based algorithm can reach dice score coefficient of 0.80 for both prostate whole gland and transition zone segmentation. Using a fine-tuning data of 115 patients from the target domain, dice score coefficient of 0.85 and 0.84 are achieved for segmentation of whole gland and transition zone, respectively, in the target domain.

Building robust deep learning-based models requires diverse training data, ideally from several sources. However, these datasets cannot be combined easily because of patient privacy concerns or regulatory hurdles, especially if medical data is involv ed. Federated learning (FL) is a way to train machine learning models without the need for centralized datasets. Each FL client trains on their local data while only sharing model parameters with a global server that aggregates the parameters from all clients. At the same time, each clients data can exhibit differences and inconsistencies due to the local variation in the patient population, imaging equipment, and acquisition protocols. Hence, the federated learned models should be able to adapt to the local particularities of a clients data. In this work, we combine FL with an AutoML technique based on local neural architecture search by training a supernet. Furthermore, we propose an adaptation scheme to allow for personalized model architectures at each FL clients site. The proposed method is evaluated on four different datasets from 3D prostate MRI and shown to improve the local models performance after adaptation through selecting an optimal path through the AutoML supernet.
Data augmentation refers to a group of techniques whose goal is to battle limited amount of available data to improve model generalization and push sample distribution toward the true distribution. While different augmentation strategies and their co mbinations have been investigated for various computer vision tasks in the context of deep learning, a specific work in the domain of medical imaging is rare and to the best of our knowledge, there has been no dedicated work on exploring the effects of various augmentation methods on the performance of deep learning models in prostate cancer detection. In this work, we have statically applied five most frequently used augmentation techniques (random rotation, horizontal flip, vertical flip, random crop, and translation) to prostate Diffusion-weighted Magnetic Resonance Imaging training dataset of 217 patients separately and evaluated the effect of each method on the accuracy of prostate cancer detection. The augmentation algorithms were applied independently to each data channel and a shallow as well as a deep Convolutional Neural Network (CNN) were trained on the five augmented sets separately. We used Area Under Receiver Operating Characteristic (ROC) curve (AUC) to evaluate the performance of the trained CNNs on a separate test set of 95 patients, using a validation set of 102 patients for finetuning. The shallow network outperformed the deep network with the best 2D slice-based AUC of 0.85 obtained by the rotation method.
We present a deep learning strategy that enables, for the first time, contrast-agnostic semantic segmentation of completely unpreprocessed brain MRI scans, without requiring additional training or fine-tuning for new modalities. Classical Bayesian me thods address this segmentation problem with unsupervised intensity models, but require significant computational resources. In contrast, learning-based methods can be fast at test time, but are sensitive to the data available at training. Our proposed learning method, SynthSeg, leverages a set of training segmentations (no intensity images required) to generate synthetic sample images of widely varying contrasts on the fly during training. These samples are produced using the generative model of the classical Bayesian segmentation framework, with randomly sampled parameters for appearance, deformation, noise, and bias field. Because each mini-batch has a different synthetic contrast, the final network is not biased towards any MRI contrast. We comprehensively evaluate our approach on four datasets comprising over 1,000 subjects and four types of MR contrast. The results show that our approach successfully segments every contrast in the data, performing slightly better than classical Bayesian segmentation, and three orders of magnitude faster. Moreover, even within the same type of MRI contrast, our strategy generalizes significantly better across datasets, compared to training using real images. Finally, we find that synthesizing a broad range of contrasts, even if unrealistic, increases the generalization of the neural network. Automated prostate segmentation in MRI is highly demanded for computer-assisted diagnosis. Recently, a variety of deep learning methods have achieved remarkable progress in this task, usually relying on large amounts of training data. Due to the natu re of scarcity for medical images, it is important to effectively aggregate data from multiple sites for robust model training, to alleviate the insufficiency of single-site samples. However, the prostate MRIs from different sites present heterogeneity due to the differences in scanners and imaging protocols, raising challenges for effective ways of aggregating multi-site data for network training. In this paper, we propose a novel multi-site network (MS-Net) for improving prostate segmentation by learning robust representations, leveraging multiple sources of data. To compensate for the inter-site heterogeneity of different MRI datasets, we develop Domain-Specific Batch Normalization layers in the network backbone, enabling the network to estimate statistics and perform feature normalization for each site separately. Considering the difficulty of capturing the shared knowledge from multiple datasets, a novel learning paradigm, i.e., Multi-site-guided Knowledge Transfer, is proposed to enhance the kernels to extract more generic representations from multi-site data. Extensive experiments on three heterogeneous prostate MRI datasets demonstrate that our MS-Net improves the performance across all datasets consistently, and outperforms state-of-the-art methods for multi-site learning.

The diagnosis of prostate cancer faces a problem with overdiagnosis that leads to damaging side effects due to unnecessary treatment. Research has shown that the use of multi-parametric magnetic resonance images to conduct biopsies can drastically he lp to mitigate the overdiagnosis, thus reducing the side effects on healthy patients. This study aims to investigate the use of deep learning techniques to explore computer-aid diagnosis based on MRI as input. Several diagnosis problems ranging from classification of lesions as being clinically significant or not to the detection and segmentation of lesions are addressed with deep learning based approaches. This thesis tackled two main problems regarding the diagnosis of prostate cancer. Firstly, XmasNet was used to conduct two large experiments on the classification of lesions. Secondly, detection and segmentation experiments were conducted, first on the prostate and afterward on the prostate cancer lesions. The former experiments explored the lesions through a two-dimensional space, while the latter explored models to work with three-dimensional inputs. For this task, the 3D models explored were the 3D U-Net and a pretrained 3D ResNet-18. A rigorous analysis of all these problems was conducted with a total of two networks, two cropping techniques, two resampling techniques, two crop sizes, five input sizes and data augmentations experimented for lesion classification. While for segmentation two models, two input sizes and data augmentations were experimented. However, while the binary classification of the clinical significance of lesions and the detection and segmentation of the prostate already achieve the desired results (0.870 AUC and 0.915 dice score respectively), the classification of the PIRADS score and the segmentation of lesions still have a large margin to improve (0.664 accuracy and 0.690 dice score respectively).
